'Palliative Care' refers to the specialist interdisciplinary clinical practice of palliative care and the designated service system funded to deliver palliative care.

Barwon South Western Palliative Care Consortium supports the following underlying principles:

  • Care is holistic, multidisciplinary and client-centred.
  • Care includes medical, nursing, allied health and volunteer services.
  • Support is provided for carers, including grief and bereavement support.
  • Clients can make informed choices about their care including the type of care and where the care is delivered.
  • Service delivery is seamless between locations where care is delivered, whether that is in the community or in a health service.
